One side fluctuates from 0.3 - 0.6V accent on or off, the other side seems to go from about 3-5V accent off, and 2.5-4.5V accent on, although it seems pretty random.
Cables are good.
Just to make sure I'm doing this right, I'm doing these tests with a sequence running with no accented notes on. To test accent on voltage I hold the accent button while checking with the multimeter.
I'll check my solder joints.

put it in keyboard mode
and press the accent button once
this should turn accents on
and then measure it again
it might be that they are cycling on and off
with each played note

so it looks like accent isnt turning off
is the accent light going off on the frontpanel
try playing a note or two
between switching accent on and off
do you hear the accent being changed

Well this sucks. The jumper cables to the headphone and mix out broke off the board. Don't have any wire at the moment so this will have to wait.
Thanks for your help so far, guest. Not sure why the accent wasn't turning off this time around. The light was on on the board. In my earlier tests it would go from 0V when on to 0.6V when off. This was with a sequence running that had no accents programmed, and me toggling accent manually by holding down the button.

you can probably strip back a bit of the old wire
and just solder that in
when its all up and running again
put it in keyboard mode and play a few notes
and listen to see if accents are on or off
and then turn them on
and repeat the test
the accent led should go on and off as well

either one is fine
now that you have a steady signal
it just depends upon wether you want a 12v or 5v control signal

But didn't you say Q35 should be 12V rather than 0.6V? 5V or 12V is fine for me, but as I said before, the digital one won't trigger everything unless I run it through another module first. I'll try it again now though. I'm guessing I can take the 12V trigger from the other side of R139 if I want to try that? The one that gave me these readings:

OK, tried the 12V trigger as well; same issue as the 5V - will only trigger certain modules unless I put it through the attenuator. I'm willing to live with this since it's not a big deal.
One thing I didn't notice though (with the 5V and 12V) is if you have two accented notes in a row, the voltage doesn't go low, so my modular sees it as one long gate signal. Is this just a limitation of the how the x0x does accents?
