help! Mintyboost not working

Hi i think I have done all the connections correctly, but my phones are not charging. The phones recognize that there is a charger but will not charge. After 5-10 minutes the batteries and the chip became extremely hot. After i took out the batteries and tested them with a DMM, they were pronounced dead. Since then, I've been trying to troubleshoot my little PCB and could not find any obvious errors. Also, I've disconnected the battery wires to take a better picture.

Did it pass all the voltage tests?
What kind of batteries are you using?
Did you turn the phone off while attempting to charge?
