Test for continuity between A3 and A4. It looks like there are some stray wire strands and solder there. There must be something that is disrupting the i2c communication.

That may be the issue. There's a short at the board. If I desolder all wires from the breakout headers I get 3 ohms between the SDA and SCL pins.

I get 3 ohms between the SDA and SCL pins.
