Cheers: It only took a couple hours to get the I-MEI code change to propagate (they said it would be 72 hours).
Jeers: But, I think I lost $30 in credit on an irretrievable sim.

I'm now on my 4th SIM card with these guys:
SIM #1: Didn't come with an activation card. There is no way to move forward without it.
SIM #2: Moved it from broken FONA #1 to FONA #2. Takes 72 hours to switch devices in their system.
SIM #3: Accidentally purchased a mini-sim (no punch-out; had to return).
SIM #4: T-Mobile website pooped out in the middle of registration; in Sim-limbo.
If you have a problem, both the chat & the phone CSRs cannot help you. They'll just read their phone script and tell you they "completely understand your problem".
I'm going to keep trying, but, gosh, these people are zero help.

SOLVED:
I was able to fix this using an online AT Command Tester:

Took me a while, but I was eventually able to find the SMS from T-Mobile there. I hope this helps anyone who needs to get a sim card out of limbo!
