Received a broken Female DC Power Adapter

Hello,
I ordered two of the Female DC Power Adapter - 2.1mm Jack to screw terminal block PID: 368.
One of them is broken. The negative (-) terminal won't accept a wire. The arm that comes down inside to capture the wire is jammed down to the bottom plate. I tried lifting it with a jewelers screwdrivers but it won't budge.
And yes, I have unscrewed the locking screws all the way...
How do I exchange this for another one or for a refund?
Also, your Contact Customer Service form on your web page doesn't work. Can't type into any of the boxes...
